The Tracker Worksheet Set is a two page set of self-monitoring worksheets consisting of The Habit Tracker and Mood Log. The Habit Tracker can be used for any type of behavioral goal you are interesting in setting for yourself, and the Mood Log can be used to monitor the emotions and feelings you may experience during that same timeframe. The Mood Log can be used with any emotion wheel (not included in this set). These are basic CBT tools to help improve awareness about the connection between your thoughts, feelings and actions.
